Peninsula Center of Cosmetic Dentistry Interviews Josh Botbol, COO of GlobalMed

Most dental practices treat virtual care as a scheduling convenience. PCCD has always treated it as something more fundamental. As the Bay Area's only Digital Smile Design certified cosmetic practice, we built our patient experience around a core belief: the journey to a new smile should be as seamless, personal, and anxiety-free as the result itself. Our virtual consultation pipeline, 3D smile previews, in-house ceramics lab, and multi-specialty team all exist in service of that belief.

When we came across GlobalMed, we recognized a company operating from the same conviction in a very different arena. GlobalMed powers the world's most advanced virtual health technology platform, a claim backed by two decades of deployment in the most demanding healthcare environments on Earth, including the White House Medical Unit, the U.S. Department of Veterans Affairs, and military operations across 60+ countries. Their infrastructure exists because they believe clinical-grade care should reach every patient, regardless of where they live. That is a philosophy PCCD understands intimately. We sat down with COO Josh Botbol to explore what it looks like when that philosophy meets cosmetic dentistry.

Q: PCCD already runs virtual consultations. Patients upload a smile photo and receive a personalized video response from one of our dentists. What does GlobalMed's platform offer that our current process does not?

Josh: Uploading a smile photo and receiving an initial call from a care provider is a solid first step in establishing care. It helps dentists get an initial impression of what might be going on with a patient. What GlobalMed offers is the ability to go beyond that first impression to get to actual treatment decisions. For example, our MouthWatch® Plus intraoral dental camera connects remote specialists to patients instantly with clinical-grade tele-dentistry imaging. That means patients don’t have to travel to one of your offices for a detailed, high-quality dental examination. Those videos and images are captured in our eNcounter® software ecosystem, where they can be stored, accessed, and shared with other care providers without toggling between different EHR systems, which means less work and stress for your team and the patient.

Q: The cases we handle (full-mouth reconstructions, complex implant placements, multi-veneer smile designs) require input from multiple specialists and our in-house ceramics team. How does a virtual platform support that kind of collaborative, precision-driven process?

Josh: With eNcounter®, the prosthodontist and the implant surgeon can be in the same virtual encounter together, seeing the same imaging and clinical data at the same time. So when they're working through a case, they're seeing the same picture from the start, not comparing notes from separate appointments.

For the ceramics team, it works a little differently. During the appointment, we're capturing images through our MouthWatch® intraoral camera and ClearCam® PTZ, and those get stored in eNcounter® so the ceramics team can pull them up on their own time. They go through the prep photos, the shade captures, the margin detail, and if they see something that needs to change before fabrication, that note goes back to the clinician. The case doesn't move forward until everyone has had a chance to weigh in.

Q: PCCD's patients are Silicon Valley professionals who evaluate enterprise technology for a living. They will see through anything that does not match the quality of care we deliver. What gives GlobalMed's platform the credibility to stand up to that scrutiny?

Josh: The question I'd ask any vendor is what the harshest environment their platform has been certified for. Ours has been cleared for the White House Medical Unit and for active military deployments in locations with no existing infrastructure. The engineering that comes out of meeting those requirements produces a different kind of platform.

A patient who evaluates enterprise technology for a living is going to look at what environments this platform has been trusted in and whether the security certifications and compliance measures hold up under real scrutiny. GlobalMed earned its DoD Authorization to Operate and a SOC 2 report, demonstrating a strong commitment to security in some of the most demanding environments. That's a different conversation than most telehealth vendors can have.

Q: Many PCCD patients have avoided dentistry for years because of anxiety. For them, that first contact with our practice carries enormous emotional weight. Can telemedicine actually serve that moment in a meaningful way?

Josh: Absolutely, and I think it serves that moment better than an in-office first visit does. Dental anxiety is fundamentally about loss of control in an unfamiliar environment. When the first contact happens through eNcounter®, the patient is in their own space, they set the pace, and they can step away the moment something feels like too much. That dynamic does not exist when someone is already in the chair.

By the time a patient like that walks into your Los Altos or Mountain View office, the anxiety profile has changed. They know your team. They have a sense of what the process will look like. The encounter has already happened in a space where they felt safe enough to show up. What eNcounter® does for a patient who has been avoiding the dentist for ten years is give them a way to take that first step without the chair being in the room. The warmth your team brings is still yours. The platform just makes it possible for the patient to be in a state where they can actually receive it.

Q: For patients who travel to PCCD from outside the Bay Area for major procedures, what does GlobalMed make possible once they return home to recover?

Josh: Right now, a patient who flies home to Denver after a major restorative case is either relying on a local provider who wasn't there or managing largely on their own. The treating clinician essentially exits the case at discharge.

With GlobalMed, the PCCD clinician who performed the procedure can stay in the case. The patient opens a consultation from home, and the clinician is evaluating healing tissue directly, using the ClearCam® PTZ to see what they need to see at the level of detail that actually matters. Post-operative protocols get adjusted based on what the clinician observes, which is a different quality of information than what any patient can accurately describe over a phone call.

Thanks to our technology, the procedure and the recovery are the same clinical case, regardless of where the patient goes home to.
